1.1 There is no entry fee to submit an EAPG Awards nomination and participation in the EAPG Awards requires no purchase.
1.2 Each nominee must be an ADHD patient advocacy group / organisation (“Organisation”) and may not be an individual / natural person (“Nominee”).
1.3 The EAPG Awards are open to all Organisations supporting people living with ADHD for projects they have undertaken in 2015 (“Project”). Only Organisations organised and operating outside of the United States of America are eligible to be nominated. Nominees which operate within the United States of America will not be considered.
1.4 Shire may at any time, in accordance with the provisions of these terms and conditions, deem a country and/or Nominee as ineligible for nomination or to receive an EAPG Award.
1.5 If Shire deems a potential Recipient (as defined in clause 5.2) ineligible to receive a grant (pursuant to these terms and conditions), Shire may donate the grant, which that disqualified Organisation would have been awarded, to another charitable organisation.
1.8 Employees of Shire plc (Shire plc, No 99854, 22 Grenville Street, St Hellier, Jersey, JE4 8PX) and its subsidiaries or their immediate families and the employees of Patient Organisations and promotional agencies directly involved in the EAPG Awards are ineligible to submit nominations.
1.9 Shire, its employees and its agents involved in the EAPG Awards shall herein be collectively referred to as the “Sponsors”.
2.1 People submitting nominations (“Nominator(s)”) must be at least eighteen (18) years of age at the time the nomination is submitted.
2.2 Nominations may be submitted via email (email@example.com) or via postal service to the EAPG Awards Secretariat (Excellence in ADHD Patient Group Awards, c/o Just:: Health Communications, River House, 37 Point Pleasant, London SW18 1NN, United Kingdom).
2.3 Nominations may be submitted from 00:00:01 AM BST on 15 June 2016 until 11:59:59 PM BST 29 August 2016.
2.4 Nominations must be in either English, or the official language of the country from where the nomination is being submitted.
2.5 Only one Organisation may be nominated per submission; multiple nominations are not acceptable.
2.6 As part of a nomination, a Nominator shall NOT disclose any specific information about medications or prescription drug products purchased, used or taken as part of the treatment of any patient involved with or represented by the nominated Organisation. In addition, the Nominator shall NOT disclose any detailed personal information about any patient represented by or involved with the Organisation.
2.7 False or deceptive nominations or acts shall render a Nominee ineligible.
2.8 On submission, all nomination forms and their contents become the property of Shire and will not be returned. Shire reserves the right to edit reasonably the contents of nominations for use in publication or promotion of the EAPG Awards.
3.1 By submitting a nomination you grant Sponsors the right, unless prohibited by law, to use the contents of your nomination (including any personal data set out therein) including your name, voice, picture and likeness, without compensation, for the purpose of processing and considering your nomination(s), advertising and publicizing all matters related to the EAPG Awards in any medium, throughout the world.
3.2 Nominators must also obtain the consent of each representative of the Nominee prior to submitting the Nominee’s or their representative’s personal data (if any) with each nomination in order to allow the Sponsors, to the extent permitted by law, to process their personal data (including their name, voice, picture and likeness), without compensation, for the purpose of processing and considering the relevant nomination(s), advertising and publicising all matters related to the EAPG Awards in any medium, throughout the world.
4.1 Nominees may be asked to provide additional information about the nature and activities of the Organisation and this information may be verified as part of the review and judging process. If such additional information is not received by the Sponsors, or if such information is not or cannot be verified, the relevant nomination may be deemed ineligible.
4.3 By registering for the EAPG Awards, the Nominator and any representative of the Nominees are consenting to this collection, storage and use of personal data and information subject to clause 4.4 below.
4.4 Nominators and any representative of the Nominees may at any point (by sending an email to firstname.lastname@example.org) (i) request copies of and/or amend any of their personal data and information collected, stored or used by the Sponsors; and/or (ii) request that their personal data and information collected, stored or used by the Sponsors be deleted and that they be removed from the nomination process.
5 Notification and Publication
5.1 Nominees may be notified by phone, email or postal service.
5.2 Recipients may be required to sign an affidavit or other documents verifying their eligibility in the EAPG Awards including, but not limited to, liability releases, tax acknowledgment forms and, except where prohibited by law, use of name and other personal data and likeness releases (“Release Documents”) and return such documents within thirty (30) days of issuance of notification.
5.3 Execution of these Release Documents will grant Sponsors the right, unless prohibited by law, to use the Recipient’s and that of its representative names, voices, likenesses and any of the material provided by them or about them without compensation, for the purpose of advertising and publicising the EAPG Awards in any medium, throughout the world.
6.1 Each Recipient of an EAPG Award will receive Ten Thousand Euros (€10,000.00), or the equivalent amount in their country’s currency.
7.1 Recipients are responsible for the reporting and payment of all taxes related to any grant received.
7.2 Additional documents may also be required from the Recipient prior to the provision of any grant.
7.3 Please consult a professional tax advisor regarding the taxation of your Organisation’s grant since special rules may apply depending on the tax status of the Organisation.
8.1 Submissions will be judged by Shire's EAPG Awards Judging Panel (comprising independent experts from the field of patient advocacy and mental health) from among all eligible nominations received (in accordance with clause 9 below). If more than one nomination is received for a particular Nominee, Shire, in its sole discretion, may consider any of the nominations for that Nominee in its judging.
8.2 The Judging Panel will be responsible for reviewing the nominations provided in accordance with the criteria defined in clause 9.1 below.
9 Judging Criteria
9.1 From the eligible nominations received, the Project will be scored by the each judge on the Judging Panel from 1-10 on each of the following 5 criteria, to determine a total score for each Project. The total score from each judge will be combined and used to rank the nominations:
The Project had a clear strategy and objectives
The Project had measurable success markers to assess achievement of objectives
The Project had a direct and positive impact on the lives of people affected by ADHD
The Project demonstrated a creative and innovative means of delivering its objectives
The Project had an evidence-based approach
9.2 The Judging Panel shall choose the Recipients based on highest ranking nominations, in accordance with clause 9.1 above. In the case of a tie, the Judging Panel will agree the Recipient without input from the Shire representative.
9.3 The Recipients will be contacted by a Shire representative and, subject to clause 9.5 below, announced on the official EAPG Awards website as well as through other media placements chosen by Shire, on or about 17 October 2016.
9.4 All Recipients are subject to verification by Shire and must meet all eligibility requirements including the execution and return of all necessary Release Documents (in accordance with clause 5.4), or they may be disqualified and deemed ineligible to receive an EAPG Award.
9.5 The decisions of the Judging Panel will be final and binding.
9.6 Whether a Nominator is identified and which Nominator is identified, if there are multiple nominations for one Organisation, is in the sole discretion of Shire.
10.1 If, for any reason, the EAPG Awards programme is not capable of running as planned, including infection by computer virus, bugs, tampering, unauthorized intervention, fraud, technical failures or any other causes beyond the control of the Sponsors which corrupt or affect the administration, security, fairness, integrity or proper conduct of the EAPG Awards, Shire reserves the right in its sole discretion to cancel the program, disqualify any individual who tampers with the entry process and to cancel, terminate, modify or suspend the EAPG Awards.
10.2 Sponsors assume no responsibility for any error, omissions, interruption, deletion, defect or delay in operation with transmission, communications, line failure, theft or destruction or unauthorized access to or allegation of submissions.
10.3 Sponsors are not liable for any problem or technical malfunction of any telephone network or lines, computer online systems, servers, equipment or software, failure of any email or entry to be received on account of technical problems or traffic congestion on the Internet or at any website, phone lines or any combination thereof, including any injury or damage to a participant’s computer related to or resulting from participation in the EAPG Awards.
10.4 CAUTION: ANY ATTEMPT BY A PARTICIPANT TO DELIBERATELY DAMAGE ANY WEBSITE OR UNDERMINE THE LEGITIMATE OPERATION OF THE EAPG AWARDS IS A VIOLATION OF CRIMINAL AND CIVIL LAWS AND SHOULD SUCH AN ATTEMPT BE MADE, SPONSORS RESERVE THE RIGHT TO SEEK DAMAGES TO THE FULLEST EXTENT PERMITTED BY LAW.
10.5 Sponsors are not liable for any late, lost, garbled, misdirected, incomplete or damaged nomination; any disruptions, injuries, losses or damages caused by events beyond the control of Sponsors; or any printing or typographical errors in any materials associated with the EAPG Awards.
10.6 Sponsors are not in any way responsible or liable for the use of the prize awarded or for any damage, loss or injury (including death) resulting from the Nominator’s or the Nominee’s participation in the EAPG Awards and/or acceptance of any grant received.

10.8 These terms and conditions will be governed by and construed in accordance with the laws of England.
